Mucinous cystadenocarcinoma (MCAC), especially bilateral disease, is exceptional and often mimics benign ovarian cysts, delaying the diagnosis.</p><p><strong>Case: </strong>We report a 16-year-old girl with hypothyroidism who presented with abdominal distension, pain, and 20-kg weight gain. Imaging showed bilateral ovarian masses with omental nodularity despite normal tumor markers. Surgery confirmed FIGO IIIC bilateral MCAC, and she completed six cycles of paclitaxel-carboplatin.</p><p><strong>Conclusion: </strong>At three‑ year follow‑ up, the patient continues to be disease‑ free. Abstract
