The effect of narrative persuasion on interpersonal talk and attitude (the case of COVID-19) 叙事说服对人际谈话和态度的影响(以COVID-19为例)

Purpose The importance of effective public messages has been widely recognized during the coronavirus disease 2019 (COVID-19) pandemic. In particular, the role of news items and interpersonal conversations for the acceptance of public health measures has been highlighted. The authors propose a conceptual model based on the existing literature on how to measure the degree of persuasion of news narratives in the context of the COVID-19 pandemic. Design/methodology/approach The authors adopted a whole population approach, where the unit of analysis was the population of the media news about the COVID-19 pandemic. The authors selected a sample to develop and test their conceptual model. The sample size was n = 248. The questionnaire was distributed online using a non-probability convenience sampling plan. The authors used a pre-post pseudo-experimental design. Respondents answered questions about their attitude toward the COVID-19 pandemic. After watching a narrative news report on the same subject, they then answered questions designed to measure changes in their attitude. A structural equation model, the Sobel test and a paired samples t -test were used to test hypotheses. Findings The results showed that there is a significant relationship between narrative with transportation and empathy. There was also a positive and significant relationship between transportation and empathy with attitude and interpersonal talk. The relationship between transportation and self-referencing was also supported. Further, transportation and attitude mediated the relationships between narrative and interpersonal talk, self-referencing as well as empathy. A paired samples t -test revealed that attitudes were changed or reinforced before and after watching the narrative news report. Originality/value This paper contributes to the body of knowledge by identifying the outcomes of narrative persuasion during public health crises such as the COVID-19 pandemic.

PurposeThis study aimed to determine the impact of telemedicine with WhatsApp on adherence to treatment plans amongst patients who have experienced oral and maxillofacial surgeries like implants, wisdom tooth removal, facial trauma and facial reconstruction.Design/methodology/approachThe researchers used a descriptive survey research design and sampled a total of 294 patients in Nigeria who experienced oral and maxillofacial surgeries 6 months before the study and provided responses that were used for analysis. The questionnaire was used as the instrument for data collection.FindingsThe researchers found that telemedicine for counselling after oral and maxillofacial surgeries have a significant relationship with adherence to the treatment plan. The analysis revealed a significant interactive effect of telemedicine competence F(3,290) 11.423, p = 001 ? p2 = 0.038. The more skilful patients are at telemedicine, the more likely they are to engage in it and vice versa. The participants also preferred a combination of live and recorded interaction forms. It was also found that a combination of different message forms, like audio, text and video, predicts satisfaction with telemedicine.Originality/valueThis study has shown that social media are now playing important roles in health care delivery to patients, and this may continue to expand as societies evolve.

Uncovering misalignments in the health- and elderly care system 揭露卫生和老年保健系统的失调

Purpose The increasing size of the elderly population is emerging as a primary catalyst for the escalation of healthcare expenditure, and a sense of urgency is manifest. However, the complexity of the health- and elderly care systems provides challenges in improving system efficiency. Hence, the system-level understanding of the main obstacles to integration care needs further exploration. In order to better integrate health- and elderly care, the study needs to identify the actual misalignments underpinning the issue. This study provides the theoretical foundations for resource misalignments and provides empirical examples of these. Design/methodology/approach Semi-structured interviews with multiple stakeholders on various hierarchical levels were carried out to create a more complete view of the system and resources deployed in health- and elderly care. The application of user-centered design methods and co-creation with employees have also been crucial to the outcomes of the study. Findings Results show that health- and elderly care is a large-scale complex system. The overlapping and mutually reinforcing misalignments are: (1) regulation and policy differences, (2) stakeholder quantity and variation, (3) external control of health- and elderly care, (3) decreasing collaboration and (4) communication channels and IT development. Originality/value This qualitative study builds on institutional theory and resource integration theory and contributes with empirical descriptions of misalignments in the health- and elderly care system. These descriptions will serve as points of departure for systems design to improve the efficiency and effectiveness of health- and elderly care.

Purpose This paper aims to provide new insights into and offer potential solutions to the challenges encountered by mental health services working with remote, rural or underserved communities in the UK. Design/methodology/approach In this paper, the authors reflect on the utility of integrating conventional clinical approaches, with preventive care and empowering work within the community, to provide culturally sensitive and accessible mental health services. The authors describe an example of community intervention from a mental health service in Ghana designed to enhance reach within remote and rural communities and identify potential lessons for practice in the UK. Findings The partnership between community mental health services and the rural communities, including families and existing social frameworks, applies collaborative care to overcome the lack of resources and facilitate the acceptability of mental health services to the local population. There are a series of important lessons from this experience including the importance of understanding the culture of a community to optimise reach and the importance of working IN the community and WITH the community. Originality/value This paper is novel because it provides learning from a model of care applied in the global south that has potential for implementation with underserved populations in the UK. The authors suggest a reframing of the notion of community care to encompass existing frameworks of community, not merely a biomedical conceptualisation.

Client perspectives on primary care integration in a rural-serving behavioral health center 客户对农村服务行为健康中心初级保健整合的看法

Purpose Advancing behavioral health and primary care integration is a priority for helping clients overcome the complex health challenges impacting healthcare deserts like those in Arizona, United States of America (USA). This study aimed to explore the perspectives of people with a substance use disorder (SUD) on accessing integrated primary care (IPC) services in a rural-serving behavioral healthcare organization in Arizona. Design/methodology/approach Clients from a behavioral health facility in Arizona (n = 10) diagnosed with SUDs who also accessed IPC participated in a 45-min semi-structured interview. Findings The authors identified six overarching themes: (1) importance of IPC for clients being treated for SUDs, (2) client low level of awareness of IPC availability at the facility, (3) strategies to increase awareness of IPC availability at the behavioral health facility, (4) cultural practices providers should consider in care integration, (5) attitudes and perceptions about the experience of accessing IPC and (6) challenges to attending IPC appointments. The authors also identified subthemes for most of the main themes. Originality/value This is the first study in rural Arizona to identify valuable insights into the experiences of people with SUDs accessing IPC, providing a foundation for future research in the region on care integration.

Purpose In the USA, integrated care remains largely aspirational; a majority of Americans receive mental health care entirely within the primary medical care outpatient setting. A new instrument, the Multidimensional Behavioral Health Screen (MBHS) ( McCord, 2020) was developed specifically to address this gap by systematically screening for a broad array of mental health issues in every patient, every visit. The goal of this current feasibility study was to evaluate the perceptions of the primary medical care providers regarding the usefulness and practicality of the MBHS, a necessary step toward broad-based implementation. Design/methodology/approach The MBHS was given to 101 consenting patients in a primary medical outpatient setting, each of whom was then seen by one of six participating primary care providers (PCPs). Providers completed a brief survey rating the ease of use, understandability, helpfulness and perceived accuracy of the MBHS after each patient visit and a final summary survey at the conclusion of the study. Findings Ratings were very positive overall, and the MBHS was clearly preferred to the traditional screening measures (Patient Health Questionnaire-9 [PHQ-9] and Generalized Anxiety Disorder-7 [GAD-7]). Providers offered suggestions for improvement and particularly for implementation. Research limitations/implications The small sample size (101 patients and 6 PCPs) and limited geographical reach may limit generalizability. Surveying providers using similar methodology should be done with larger numbers of providers and more diverse primary medical care settings. Practical implications This study provides evidence that the MBHS may be significantly effective in operationalizing the integrated care model in United States (US) healthcare systems. Social implications The MBHS, a new behavioral health screening tool, was perceived by providers as useful in identifying mental health issues and guiding treatment decisions in the primary care setting. Originality/value This paper identifies a novel screening instrument that implements new and emerging models of psychological dysfunction in a practical way in primary medical care, making integrated care a reality rather than an aspiration.

Purpose The new Italian National Recovery and Resilience Plan (NRRP) has prioritised a new healthcare model that will establish the additional community healthcare facilities (CHFs). The paper proposes a methodology for supporting decision-making on location of the future facilities according to new parameters that consider how proximity to healthcare benefits communities. Rethinking the spatial parameters for locating future CHFs, focusing on fragile areas, creates a novel decision support system. Design/methodology/approach The methodology is based on multifactor analysis and on geographic information system (GIS) mapping to simulate the potential and risks associated with the proposed location of CHFs, focusing on territorial contexts of metropolitan cities, medium-sized cities, and Inner Areas, characterized by different fragilities. This method aims to innovate urban planning practices by updating the practice of per-capita urban planning standards and promoting implementation of the 15-minute city model. Findings The method defines new spatial parameters useful to inform the appropriate location of CHFs in Italy's Inner Areas. This offers a new integrated approach to spatial design mixing urban planning and healthcare dimensions. Originality/value The methodology will bring about an integrated urban planning approach, which guides both transformative urban choices and health services' implementation according to the needs of communities.

Factors influencing the perception and the willingness to take the COVID-19 vaccine among the working population in Ghana 加纳劳动人口对COVID-19疫苗认知和接种意愿的影响因素

Purpose The onset of the coronavirus disease 2019 (COVID-19) has led to loss of lives and jobs and other adverse effects on persons, families and businesses. With its frightening tendencies, it was a sigh of relief with the discovery of vaccines to mitigate its disastrous effects. However, participation in the vaccination exercise in Ghana has been slow and unencoraging. This paper sought to identify the factors that influence the perception and willingness of individuals to receive the COVID-19 vaccine. Design/methodology/approach For this study, structured questionnaires were administered to 1,350 respondents selected at random by way of purposive sampling from specific regions that were also randomly selected. Out of this sample, a total of 1,296 were found to be in useable form. The probit model estimation with marginal effects was used to determine both perception and willingness to take the COVID-19 vaccine. Findings The authors observed that increases in age and manufacturing sector workers were positively associated with vaccine perception, while both males and females residing in the Bono and Central regions had negative perceptions toward the vaccine. Among male workers, education and living in Accra had a negative relationship with vaccine perception, while for female workers, education had a positive influence on vaccine perception. Additionally, workers in the transport sector and those residing in urban areas were more inclined to receive the COVID-19 vaccine in Ghana, regardless of gender. Again, the authors observed that larger household size, people with no formal education and females with primary education were more willing. Also, increase in age for males reduced willingness, while residents in the greater Accra region were less willing to take the vaccine. Originality/value To the best of the authors' belief and knowledge, the observations and recommendations made are consistent with theory and empirical studies and contribute immensely to the discussions about the most effective ways of combating the COVID-19 pandemic. It also offers a nuanced perspective on how policy-makers can enact policies that ensure efficient and effective ways of dealing with future epidemics.

PurposeHealth care integration is crucial in improving service equality and patient outcomes. However, measuring integration between the health and social care sectors remains challenging. This article aims to review existing systematic models to identify alternative health and social care integration measurement tools. The review focuses on models that involve systematic planning and long-term cooperation across different organizational sectors.Design/methodology/approachThe study examines various dimensions and elements of integration, including process, outcome and structural measures. It compares different tools used to measure social and health care integration, such as the Rainbow model, Balanced Scorecard (BSC) Scorecard, PRISMA, SCIROCCO, integRATE, health-data simulation (HSIM) and the model developed by Åhgren and Axelsson. The analysis includes both empirical studies and theoretical frameworks.FindingsThe findings highlight the importance of standardized measurement methods to assess the impact of integration initiatives on patient outcomes, healthcare costs and the quality of care.Originality/valueThe review contributes to the ongoing discourse on social and health care integration, particularly in the Nordic context. The results can inform social and healthcare providers, policymakers and researchers in evaluating and improving integration initiatives.

PurposePopulation Health Management (PHM) is a methodology which has the potential to help support the aspirations of the NHS's Integrated Care Systems to address health inequalities, shift from reactive to proactive care and prevention, work in a more integrated manner, and deliver person-centered care tailored to the individual's needs. Although PHM is featured in a number of key national policy documents, moving from the conceptual to widespread adoption requires some key conditions to be in place and barriers to be overcome. It may also require a wider cultural change amongst teams as part of a more truly integrated, holistic ethos.Design/methodology/approachThis Viewpoint Paper sets out the advantages and challenges associated with the adoption of PHM.FindingsKey features of PHM are outlined, as well as the conditions needed for successful adoption. Key barriers which need to be overcome are discussed, not least occasional scepticism from clinical colleagues and how this can be addressed and translated into a source of energy and enthusiasm for a new approach.Originality/valueThis is a viewpoint based on the author's personal experience of supporting the implementation and spread of PHM in England's NHS, summarising shared learning and suggesting practical strategies.
