The Haag-Streit Lotmar Visometer™ was evaluated to determine its accuracy in predicting macular potential prior to cataract removal in 101 cases. Overall, the instrument was reasonably reliable in 73% of cases. The results were analyzed from several standpoints to determine what data obtained during routine preoperative examination might help predict the reliability of the instrument in a given case.

The various types of posterior chamber intraocular lens malpositions, in increasing degrees of severity, are described. These include pupillary capture, optic decentration, malpositioned loop, windshield wiper, sunrise, sunset and lost lens syndromes. The etiologies, management, and prevention of these complications are discussed.

Seven days after extracapsular cataract extraction and intraocular lens implantation, a posterior chamber lens was removed following the patient's sudden death. The lens was studied with the implant cytology technique, which demonstrated a continuous and optically clear separating film of proteinaceous nature. The film was fully developed and covered the entire implant. A few macrophages were found in one area on the surface of the otherwise acellular film; these were involved in blood phagocytosis and did not appear to have been involved in the formation of the acellular proteinaceous film.
