Objectives

The aim of this study is to evaluate the accuracy of a new guided surgery system.

Materials and methods

Twenty-six patients were treated in 8 centers with a total 116 implants placed. The surgical guides were bone supported, mucosal or teeth supported. At the end of the surgical phase, a post-op CT scan was taken to evaluate the discrepancies between the virtual and the clinical implant position.

Results

The mean values regarding the 3D implant position are in line with the mean values reported in the literature. Bone supported and mucosal supported guides are more precise than dental or dental-mucosal supported guides, however, the differences are not statistically significant. Discrepancies at the implant apex, at platform level and at the angle between virtual and clinical position were evaluated by superimposing a pre-op CT scan to a post-op CT scan.

Conclusions

The accuracy of the guided surgery system used is in line with the data of the literature. A good diagnostic phase is recommended and it is very important to plan the implant position with a safety distance from the anatomical structures of at least 2 mm. In clinical cases of immediate loading it is recommended to reline a provisional prosthesis to compensate the discrepancies between the virtual and the clinical implant position.

Objectives

Review surgical devices already introduced in plastic-reconstructive surgery in the field of dental care, i.e. tissue expanders.

Materials and methods

Expanders have been used in oral surgery for several years to address the problem of soft tissue management in bone regeneration techniques. Expanders with valve and idrogel self-inflating were identified. Features, advantages, disadvantages, and directions to use them were described.

Results

Several animal studies suggested a sub-periosteal placement of the device, whereas only one study on human suggested an over-periosteal position. There are controversial opinions about bone resorption associated with them.

Conclusions

Tissues expanders can be a valuable aid in the management of soft tissues; however, attention is required because of the considerable number of complications.

Objectives

To suggest a novel technique for partially or totally edentulous upper jaw rehabilitation.

Materials and methods

We describe the possibility of extending tilted implants indications with maxillary sinus membrane distalization aimed at maximizing A-P spread in those cases with mesially extended maxillary sinus.

Results and conclusions

Even if the number of treated patients is small, preliminary results suggest this could be a reliable clinical option.

Objectives

Oroantral fistulas consist of osteomucosal communications between the oral cavity and the maxillary sinus, which do not usually resolve spontaneously. In most cases, surgery is performed via a transoral approach and the fistula is closed with local flaps, but the results are often unsatisfactory. This case report shows that proper management of these fistulas depends largely on the functional integrity of the ostiomeatal complex.

Materials and methods

We present the case of a patient with an oroantral fistula. Several transoral attempts to close the fistula had failed. The endoscopic-radiological work-up revealed blockage of the ostiomeatal complex, so the patient underwent functional endonasal endoscopy.

Results

The procedure was effective in rehabilitating the ostiomeatal complex, and spontaneous closure of the oroantral fistula was observed three months after surgery.

Conclusions

This experience suggests that successful management of an oroantral fistula requires a complete multidisciplinary work-up (radiological, ENT, odontoiatric) before transoral surgical closure is attempted.

Objectives

Analyze the guidelines reported in the literature for the use of pre-operative radiologic exams in dental implantology.

Materials and methods

Articles regarding the radiologic surveys used in the evaluation of patients with prosthetic implants, published in Medline^® and/or Embase^® after 2000, were selected for analysis. Articles which considered only the accuracy and/or the radiation dose related to the use of these diagnostic methodologies (Endo-oral X-ray, orthopantomography, cross-sectional imaging) were not considered. Only the articles that evaluated, in addition to the accuracy and the radiation dose related to the radiologic examination, also their indication of use were included. A textbook reporting the inclusion criteria was also included.

Results

The searches with the keywords “CT dental implants”, “CT dental implant orthopantomography”, “dental implant radiography guidelines” and “dental implant radiology guidelines” yield 853, 106, 27 and 29 results, respectively. According to the inclusion criteria 17 articles were selected for further analysis.

Conclusions

No precise guidelines on the selection and evaluation of pre-operative radiologic exams to use in implantology were found. While some authors reserve the use of cross-sectional imaging to selected cases, others advice its use in all cases due to the high amount of information that can be obtained on both the morphology of the bone as well as its connections with other anatomical structures. However, in order to establish common guidelines further research is necessary.

Nuovi approcci chirurgici nella terapia del sorriso gengivale nell’adulto 成人牙龈微笑疗法的新手术方法

Objectives

To present a new surgical approach to gummy smile's therapy in adults, analyzing past techniques.

Materials and methods

With a review of scientific literature on the subject we analyzed every aspect of the problem “gummy smile” with the aim of presenting a new surgical combined approach in two clinical cases.

Results and conclusions

The gummy smile defect has always been a challenge to solve. If in childhood an orthodontic approach is correct, only two ways are available for adults: medical (botox) or surgical. The innovative approach of combining several techniques of the past for the gummy smile surgical therapy in adults scores a winning goal in terms of quickness in reaching the results, low morbidity, and down-time for the patient and nowadays it is seems to be the only way to get stable results.
