Objectives

The Total Face Approach is a diagnostic evaluation that gives a comprehensive vision of the patient's parameters candidate to complex treatments in implant dentistry, orthodontics or maxillo-facial surgery. Such assessment needs a cephalometric 3D evaluation whose interpretation allows to formulate a correct diagnosis and an adequate treatment plan. The aim of this paper is to introduce the scientific community with a 3D cephalometric model for diagnostic purposes in orthodontics, maxillo-facial and implant surgery and to illustrate the potential of the latest 3D technologies.

Materials and methods

To develop this cephalometric analysis, the intermaxillary skeletal relationships of 30 patients to whom a CBCT examination was prescribed for diagnostic purposes are presented. An approval of the Ethical Committee was not requested since the clinicians did not do anything different from a normal routine case in which a CBCT examination is requested instead of multiple 2D radiologic examinations. The analyses have been studied using a dedicated software (Materialise Simplant OMS, Leuven, Belgium).

Results

The radiation dose and the exposure time influence the quality of the examination. Comparing CBCT to conventional CT scans, CBCT could become the examination of choice to lower the radiation dose. It is easily accessible and has an advantageous cost-benefit ratio.

Conclusions

It can thus be concluded that our proposal of 3D cephalometric analysis can be defined a useful diagnostic tool. In all clinical cases of orthodontic surgery, maxillo-facial and implant surgery which request a more detailed evaluation of skeletal parameters regarding the entire maxillo-facial area.

Tumore a cellule granulari della lingua: due casi clinici trattati con laser 舌粒细胞肿瘤:两个用激光治疗的临床病例

Objectives

To describe the surgical treatment of two cases of Granular Cell Tumor (GCT), a benign neoplasm, whose origin still remains controversial. When involving the maxillo-facial area, tongue is the most affected organ.

Materials and methods

The excision of tumors was performed using a potassium–titanium–phosphate (KTP) laser, also known as doubled frequency neodimium:yttrium–aluminium–garnet (Nd:YAG).

Results

In both cases an excellent healing was achieved without relapses. A histological diagnosis was possible in both cases. No incidents or intraoperatory difficulties occurred nor complications in the post-operative period.

Conclusions

These lasers can be considered as the gold standard devices in the treatment of this disease when it occurs in oral soft tissues.

目的探讨两例良性肿瘤颗粒细胞瘤(GCT)的手术治疗，其起源仍有争议。当涉及颌面区时，舌头是最受影响的器官。材料和方法使用磷酸钛钾(KTP)激光，也称为双频钕钇铝石榴石(Nd:YAG)进行肿瘤切除。结果两例患者均治愈良好，无复发。两例均可进行组织学诊断。术后无意外事件、术中困难及并发症发生。结论这些激光可作为治疗口腔软组织病变的金标准装置。
