The Authors, in a review of 162 patients submitted to mastectomy for cancer, have considered the incidence of local recurrences related to the type of operation and to the clinical stage of primitive neoplasia. Particularly, the percentage of local recurrences does not seem to be related to the type of surgical procedure employed but rather to the presence of metastatic lymph nodes at the time of the first treatment of neoplasia. Is also related the treatment of the local recurrences observed and the detained data are compared with that reported in the literature, particularly about the mechanisms of formation, the incidence and the types of treatment.

The paper is dedicated to the description of a technique now new but also not very used, of regional anesthesia, indicated for surgery of upper limbs: the interscalene brachial plexus block. In Author's experience this type of anesthesia has many indications, related especially to orthopedic and traumatologic surgery, in patients with high anesthesiologic danger. This type of regional anesthesia by interscalene technique is, in Author's opinion, much better than the axillar technique, now moore frequently used.

The authors have determined, by ionic resin exchange chromatographic method, HbA1 values in 16 chemical gestational diabetic pregnant, in 4 case with insulin dependent diabetes and 34 normal pregnant women. Statistical differences were found between normal's HbA1 mean values and clinical diabetics' but not with chemical gestational diabetics. HbA1 mean values in chemical gestational diabetics with diabetic familiarity were found statistically higher than normal subjects' values. Mothers with chemical gestational diabetic, who delivered newborn with less than 50th centile had HbA1 values significantly elevated.

In order to study the suprathreshold hearing phenomena for a differential diagnosis of sensorineural hearing losses Békésy Audiometry has been used at two different levels: a) Békésy threshold level; b) Békésy comfortable level. Are considered: the tracing's amplitude; the continuous decay; the gap between the two different threshold levels. Results has demonstrated that Békésy comfortable level give substantial contribution as decay test and as indirect recruitment test.

The most recent data concerning the characteristics of hepatitis A virus, as well as epidemiology and prophylaxis of type A hepatitis are described. An epidemic of hepatitis A which occurred in a village near Parma is analysed. The diagnosis was based on the detection, by radioimmunoassay, of specific antibody to hepatitis A antigen (anti-HAV) at the beginning of the infection and during the convalescence. In addition the occurrence of high levels of specific immunoglobulins type M anti HAV, during the acute phase, was a further evidence of the diagnosis. The presence of other agents responsible for acute hepatitis, such as Cytomegalovirus and Epstein-Barr virus, has been excluded by laboratory examination.

A retrospective study was carried out on the incidence of accidental poisoning (A.P.) for children admitted to the Pediatric Clinic of the University of Parma between January Ist 1973 and December 31st 1978. A total of 218 cases of AP, equal to 2,2% of total number of admittances, were registered in this time period. The children were aged from 2 months to 13 years. The toxic substances were divided into two categories: medicinal and household products. The AP tend to increase in Summer (29,8%) and in Winter (30,7%). In this series there was only one fatal outcome (0,5%). Some norms for prevention are considered.

Two poststenotic subclavian artery dilatations are described. The distinction between a poststenotic dilatation and aneurysm is based on arteriography, showing associated with explorative arteriotomy, if needed. In the case of even wide poststenotic dilatation, the first rib only should be removed. The first rib resection in the described patients controlled after 2 and 7 years, was followed by reduction of arterial dilatation in one case and by stabilization in the other. This result confirms that arterial grafting must be considered only in the treatment of true poststenotic aneurysm.

Fine structure of pineal gland was studied in some Chiroptera and compared with those of other micromammals. Three cells types were found in pineal parenchyma: pinealocytes, the most numerous cells, glial cells and Mastzellen. Some nervous cells were found in peripheral dorsal pineal gland. Pinealocytes show a clear or dark matrix which are different functional stages of the same cell. A large nucleus with irregular profiles and deep invaginations, several mitochondria and ribosomes, canalicular rough surfaced endoplasmic reticulum, large Golgi apparatus, lipid droplets, secretory and heterogeneous granules distinguish this cellular type. Centrioles often joined with a striated root (formed by alternating clear and dark discs), cytoplasmic process extending in the intercellular space as far as 3-4 mu including cilia showing a 9+0 pattern, osmiophilic vacuoles involved in secretion processes were observed too. Glial cells irregularly arranged tend to form small clusters: they show a dense and glycogen particles rich matrix. This matrix was very positive for non specific esterases reaction. Blood vessels are provided by a continuous endothelium resting on basal lamina, surrounded by a thick perivascular layer. Collagen fibers, pinealocytes processes, non-myelinated nerve fibers and Mastzellen were found in pervascular area.
