Background: The coronavirus disease 19 (COVID-19) is a highly transmittable and pathogenic viral infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The characteristics of laboratory findings of coronavirus are of great significance for clinical diagnosis and treatment. This study aimed to evaluate the accuracy of biochemical parameters in COVID-19 and to compare them according to the severity of the disease. Methods: This diagnostic accuracy study was conducted in COVID-19 patients, During the study period 169 COVID-19 patients were hospitalized at the Avicenne military hospital, 20 patients in the medical intensive care unit (severe group) and 149 patients in the COVID-19 isolation hospital (non-severe group). Results: Through the analysis of laboratory data of patients with COVID-19 (169 patients), we demonstrated that severe cases possessed higher levels of C-Reactive Protein (CRP), Aspartate Aminotransferase(AST), Alanine Aminotransferase (ALT), and Urea levels in serum, creatinine, hypoalbuminemia, hyponatremia, hyperbilirubinemia, troponin, procalcitonin. Conclusion: The analysis of the current scientific literature demonstrates the value of laboratory parameters as simple, rapid, and cost-effective biomarkers in COVID-19 patients.

The coronavirus disease is a communicable and contagious respiratory disease that has not only affected human social lives, interactions and relationships but has equally changed other significant patterns and aspects of people’s normal life, even after the global lockdown. Nigerians, during and after the lockdown caused by COVID-19 are behaviourally influenced and affected by the novel virus. Thus, this study sought to investigate how COVID-19 pandemic has affected social lifestyle and interactions of Nigerians. The main objectives of this study were to ascertain how COVID-19 pandemic affected human communication and interactions; people’s social lifestyles and activities; people’s movement and transportation; the education system in Nigeria and house hold welfare or economy of Nigerians. The study adopted descriptive survey design with questionnaire and published materials on COVID-19 as instruments for data collection. Nigerians aged 18 years and above were studied through an online questionnaire survey which was distributed through social media platforms, emails and professional networks between 10th and 30th January, 2021. Purposive sampling technique was used to select the five most affected COVID-19 States in Nigeria in the year 2020 A total of 400 Nigerian participants completed the structured validated Likert-Scale questionnaire that assessed the impacts of COVID-19 on people’s social lives, interactions and welfare. Data were analyzed using 4-point like rt scale in descending order of SA, A, D and SD with mean and standard deviation to determine the level of responses. Major findings revealed that COVID-19 negatively affected the lives of people, ranging from lack of face-to-face communication and interaction, to change in people’s social welfare and lifestyle, to halt in education and academic activities, restriction in movement and transportation and household welfare challenges. It is suggested that policymakers should intensify efforts to ensure that corona virus is completely eradicated by providing and making COVID-19 drugs and vaccines available to all Nigerians. Government should ensure strict compliance to PTF directives on wearing of facemask by all Nigerians.

Background: Sepsis has high mortality globally. The key factor in decreasing the mortality is early diagnosis and the initiation of appropriate treatment within hours. Various biomarkers are in use for timely diagnosis but none is without limitation. It’s imperative to predict the mortality early in order to sensitize those involved in the care. We studied Neutrophil to Lymphocyte Ratio (NLR) as a prognostic marker in sepsis. Methodology: It was a retrospective study, conducted at Aga khan university hospital from 30th November 2020 to 31st March 2021. Data was calculated from patient medical record after approval from ERC. Multiple linear regression technique was used to determine the association between increased NLR with mortality. Results: Our study included 168 patients. The median (IQR) NLR of the patients was 10.4 (13.4). The mean SOFA score of the patients was noted to be 5.7 ± 2.9, mortality was reported in 26.1% (n=44) patients. Multiple linear regression showed NLR increased by 2.0 times with every mg/dl increased in average total bilirubin level (95% CI: 0.4-3.2), and by 1 times with every one unit increase in SOFA score (95% CI: 0.1-1.5). However, there was no significant association found between NLR and mortality rate. Conclusion: NLR is an effective marker for predicting sepsis; however, its role in predicting mortality is yet to be established.

Immune Thrombocytopenic Purpura (ITP) is an autoimmune disorder with isolated thrombocytopenia marked by peripheral blood platelet count<100 × 109/L without any other etiology or disorders that may be associated with thrombocytopenia. The treatment of newly diagnosed ITP patients is steroids. However, about one-third of patients do not respond to steroids. Here, we present a 63-yearold hypothyroid, hypertensive, and diabetic female presented with a history of bruising of skin in multiple sites.

Non-convulsive status epilepticus has traditionally been thought of as a rare condition. There is little data regarding its true incidence but evidence suggests it is far more common than once believed. Non-convulsive status epilepticus can have an array of presentations and can be a diagnostic enigma for those unaware of its existence or those who believe, as it was once taught, that this condition is isolated to critically ill patients in the intensive care unit. Herein, we discuss the case of a patient with a focal traumatic brain injury who was found to be in Non-convulsive status epilepticus. This condition is one that physicians need to consider, especially those physicians caring for patients with traumatic brain injury.

The nasocardiac reflex is a little known reaction of nasomucosal origin. The nasocardiac reflex itself may lead to severe bradycardia or can even result in cardiac arrest following irritation or stimulation of the nerves in the nasal cavities or paranasal sinuses. We highlight an event whereby nasocardiac reflex was observed in a patient during a common examination, which was the nasoendoscopy, manifesting as bradycardia and hypotension. After resuscitation, the vital signs improved. He was closely monitored in the ward and able to discharge home well on the same day after eight-hour of monitoring.

Background: During SARS-CoV-1 and Middle East Respiratory Distress Syndrome (MERS) outbreaks, a particularly elevated incidence of cardiovascular disease among patients was observed. With COVID-19, this correlation becomes evident again. However, the cardiovascular impacts by COVID-19 pandemic are not yet well established although there are constant publications about its potential deleterious effects. Thus, we aimed to carry a systematic review of the literature with meta-analysis based on the following guiding question: what practical contributions to the scientific literature produced in the period of 2019-2020 does the impact of the COVID-19 on cardiovascular system have to offer? Methods: A systematic review of the literature using the Virtual Health Library (VHL) and PubMed with the following descriptors: #1 "cardiovascular disease" [MeSH] AND #2 "COVID-19" [keyword], as well as their equivalents in the Portuguese and Spanish language, during the period from December 2019 to March 2020 was performed. Results: one hundred articles are found in Pubmed and twenty-seven are selected. In VHL there were 59 articles and four are selected totaling thirty-one papers. The findings were then divided into three subcategories: Etiology, Physiopathology and Risk factors of SARS-CoV-2 in Cardiovascular System; Clinical presentation, laboratory markers and imagenological aspects of SARS-CoV-2 in cardiovascular system; and Anti-Hypertensive Drugs, Cardiovascular System and SARS-CoV-2. Conclusion: It comes to the cardiovascular system, these issues are aggravated and urge as a joint commitment from researchers, medical and governmental organizations for carry out more robust studies with bold methodologies aimed at mapping prognostic factors and assertive therapeutic approaches in the management of cardiovascular complications of COVID-19.
